Periodontal Disease Treatment

Gum Surgery

Surgery of any type (especially dental) sends chills up the spine. However, cosmetic gum surgery performed by a qualified cosmetic dentistry expert is relatively simple and can greatly improve the appearance of your smile.

Cosmetic surgery is mainly the solution to two types of problems: the “long tooth” look and the “gummy smile”. Both of these conditions are usually caused by aging, gum disease or poor oral hygiene. They progressively grow worse with age. Cosmetic gum surgery can also help people with collapsed gums in the area of a missing tooth, blackened areas between implants, crowns or other teeth and tooth sensitivity caused by receding gums.

The “long tooth” look is caused by receding gums or a bad bite. As we age, our gums naturally shrink and recede. This can leave your teeth overexposed at the gum line.

The “gummy smile” is caused by excess gum tissue, which causes the teeth to appear too short. People with a gummy smile often feel self-conscious about the amount of gum tissue visible when they smile.

How is Cosmetic Gum Surgery Performed?

The two main types of cosmetic gum surgery, used to correct the long tooth look and the gummy smile, differ. The gummy smile corrective surgery reduces the excess gum and bone. The cosmetic dentist typically uses a laser to perform this type of surgery. A soft-tissue laser removes excess gum tissue while a specialized laser cuts away excess bone.

The procedure for correcting the long tooth look works in much the same way as a skin graft. Your cosmetic dentist removes healthy skin from the roof of the mouth and grafts it onto the gum line. The result is a natural looking, reinforced gum line.

Before undergoing any type of cosmetic gum surgery, discuss the procedure thoroughly with your cosmetic dentist. This procedure may be just one aspect of an overall plan with several components to improve your smile. Cosmetic gum surgery might be complimented with the use of veneers, bridges, crowns or other cosmetic dentistry treatments to get your smile looking its best. This might require more than one cosmetic dentist if all of the procedures are to be completed at once.

Will I Be Awake for This Procedure?

You need to discuss this question with your cosmetic dentist. There are two possibilities:

1. You may have the surgery done in a series of office visits. The cosmetic dentist will use a local anaesthetic each time to freeze the area of the gums they are working on. This is a good possibility with the laser surgery used to correct the gummy smile. Each visit could see more gum and bone tissue removed.

2. You might be given a general anaesthetic and have all of the work performed at once. This may be necessary if you are having grafting done from the roof of your mouth to correct a long tooth smile. If you require other work such as dental implants or preparation for veneers, have all of the work done while you under anaesthetic. There are risks every time you use a general anaesthetic so avoid having more than one done for dental work.

How Long Will It Take To Recover From Cosmetic Gum Surgery?

The speed of your recovery depends on your overall health and the extent of the surgical work. However, there are important steps you can take to aid in a healthy recover.

Have a responsible person accompany you to the appointment. This is necessary is you are having a general anesthetic, but also wise if you are being frozen and taking painkillers. Follow your cosmetic dentist’s after-care instructions carefully. They will probably recommend that you use an antibacterial mouthwash for at least two weeks after the surgery 00004000 to prevent infections. Ask the cosmetic dentist or their hygienist to demonstrate how you should brush your teeth until your gums are healed.

Plan on eating a liquid/soft diet for several days after the surgery. Soups, oatmeal, yogurt and other nutritious choices will keep you feeling strong, help you heal faster and shouldn’t irritate the gums. Avoid spicy or acidic foods such as salsa, which could inflame your sensitive post-surgery gums.

How Much Will This Cost Me?

The cost of the surgery depends on the amount of work you are having done. If you need two or more cosmetic dentists to perform several procedures at once, expect the cost to be greater. General anaesthetic will also cost extra as it must be performed by a qualified anaesthesiologist.

As with any surgery, price should not be your primary consideration. Concentrate on finding the best qualified and most experienced cosmetic dentistry professionals to perform the work. View their portfolios, call their references and ask questions until you are satisfied that you know everything you need to know about the procedure.

Once you have your plan in place, work on funding the surgery. Some people have saved for the procedure, as it is a cosmetic issue that has greatly affected their self-confidence and they can’t wait to have it done. Others might have suffered sudden damage or extreme tooth sensitivity that makes the surgery necessary but out of their price range. Work with your cosmetic dentist to find a solution through financing, payment plans, a loan or a line of credit.

Take care of your gums in the months and years after surgery by brushing, flossing and visiting the dentist for check-ups regularly. Work hard to prevent gingivitis and other periodontal diseases that may make it necessary to undergo gum surgery again in the future.

  1. QUESTION:
    gum surgery?
i havce braces and really swollen upper gums. it is not due to bad brushing tho, it just happened because of the braces. my orthodontist and dentist said that when i get my braces off i might have to have a surgery to reduce the gum size. has anyone ever had this done? does it hurt much or is it just a simple procedure?

  2. QUESTION:
    Gum Surgery?
i’m just wondering i’m in grade 9 and i have to get gum surgery. I was wondering, does it look weird when you get it done? and would i be able to go to school the next day because i have a unit test and will get 0 if i dont go? main question tho is if it looks weird when it’s done and how long it takes to get your gums/teeth to look normal again.

    • ANSWER:
if your talking about receding gums and there graphing skin on.i had that done in fifth grade.im not going to lie it does hurt not horrible like cutting off something though like dislocating a shoulder. at first the stitching looks weird when there done but noone can see unless you pull down your lip and purposly show them. you will have light skin too in that spot but it wont be noticable unless you purposely show someone too. the surgery is easy and painless it just feels like pressure.i would ask your parents too ask the doctor for your prescription though before you get the surgery because you have to wait an hour before you can get your medicine and the medicine the doctor gives you wears off like ten minutes after surgery ends. you should probably by a tub of ice cream because it hurts worse when you open your mouth farther.trust me though its not that bad.it might not even be as bad as i remember becaus i was just a fifth grader now im a eighth grader.anyways if you dont get it done your teeth will fall out. this is your fault for not brushing good.

  3. QUESTION:
How long will the results from my gum surgery last?
I just had gum surgery last weekend at my dentist’s office to repair a “gummy smile”. He used a laser to cut away some of my gums. He also said he was going to cut away some bone, but I’m not sure if he did that or not because no drills or anything like that was used. I forgot to ask him this question, so I was wondering how long do the results from this surgery last? Will my gums grow back? Because this was a very expensive surgery! Thanks!
Maybe to be more specific, I wanted to know if the gums will grow back to how it was originally (meaning my “gummy smile” will return)?

    • ANSWER:
      THIS IS AN EXCELLENT QUESTION.
AS A RULE, THE GUMS COVER THE BONE. IF THERE WAS AN OVERABUNDANCE OF BONE IN A PARTICULAR SITE, THEN TRIMMING AWAY SOME OF IT WILL HELP THE SMILE TO BE LESS GUMMY.
TAKING CERTAIN MEDICATIONS LIKE PHENYTOIN CAN CAUSE YOUR GUMS T BE SWOLLEN.
      ASK YOUR DENTIST.

Or does the depth eventually go back to 6 millimeters a few years after the surgery?

I went to the dentist because I thought I had a cavity near my gum line, but it is actually the root of my tooth beginning to show. It is not sensitive yet. Typically how long does one have until they need surgery when the root first starts showing?

    • ANSWER:
You will not necessarily ever need surgery. If only a small amount of root is showing, your only concern may be with increased sensitivity to cold. You may be required to have surgery eventually if the recession continues to worsen, and the amount of attached tissue is minimal. Typically recession can be caused by several things: Brushing too hard (use a soft bristled brush), Clenching and grinding your teeth, acidic foods, and genetics.

  15. QUESTION:
Does anyone have more information on gum disease and surgery?
I did not have dental insurance for the past 3 years, and missed dental visits. Prior to that, dentists had assured me that I had teeth in fairly decent health.

I have insurance now and I visited a dentist recently. They seemed to jump on my case the moment I said I’d missed appointments for 3 years. The staff took x-ray records at first and when the doctor walked in, the first thing he said without even looking into my mouth was that I have gum disease, and referred me to a periodontist. I was trying to understand what he was trying to say but before I understood anything, the doctor walked out of the room.

Today, I went to the periodontist who spent 10 mins looking at my teeth measuring gaps, and said that I will need to knock off two wisdom teeth and go through gum surgery. I was more aggressive in asking pointed questions and he at least answered them. He says my gums have moderate level of infection and will need to be opened up for cleaning and then stitched back.
He also prescribed a thorough dental clean-up process, which will cost me 0(my copay). The extractions and surgery are likely to run into a few thousands, not to mention the pain.
While the dentists may be right, I somehow felt that they are more focussed on business development for themselves than actually attending to my needs. E.g. my wisdom teeth are perfectly fine and healthy, and have never given me any trouble. When I asked why the extraction was needed, I was told that the teeth are deep inside & difficult to clean, so I would find it easier to maintain the health of my mouth without them. They were just keen to get me to sign up for the extraction appointment. I had to be my assertive best to say that I needed to think about it.
I’m considering seeing a couple more dentists. While I’m surely concerned about my oral health, I don’t want to be pushed into urgent and immediate procedures, which are painful & costly.
Am I being pushed into unneccessary procedures? Please help.

    • ANSWER:
You should seek a second opinion. No one should tell you that you need surgery before you have had a deep cleaning or scaling and root planning. Many of my patients have improved tremendously with the deep cleaning and oral hygiene instruction without ever needing surgery. Plus you need time to learn how to properly take care of your teeth before jumping into surgery. No sense in spending money if you don’t know how to maintain them.

We start our patients with an hour long examination, where we explain what we see and give you a treatment plan – or game plan on how to take care of the disease. We schedule you for the deep cleaning, which may take multiple visits, and then wait 4 to 6 weeks to see how the tissue has healed. At the re-evaluation visit, if there hasn’t been significant improvement and there are other factors like pus and bleeding on probing, we might discuss surgery.

However, since you took a three year break from dentistry and regular cleanings, I usually recommend getting your teeth cleaned every three months for several times after the deep cleaning before recommending surgery. That way we know that you are as good as you are going to get without surgical intervention. Plus you need to have your general dentist or restorative dentist give you a proper examination. Open contacts (or space between the teeth) are a large determinant if a pocket is going to heal. If your floss does not snap but falls into the gum tissue, everytime you eat food it is being forced up into the gum. This is called food impaction and it can over time cause pocketing and bone loss to occur around the teeth.

As for the wisdom teeth, it all depends on their position and what they look like. If they are not fully erupted and only part of the teeth are sticking out, you might want to have them removed at some point because they are a reservoir for bacteria to breed and spread forward to other teeth. It sounds as if priority number one is to start with the cleaning first, then consider your options once you know how your body will heal.

People take breaks from dentistry all the time, it is very expensive and time consuming. You maybe had other things (like feeding and clothing children) that your money had to go to at the time. You should never let a dentist make you feel bad for taking care of other issues first. You should respond with, the past is the past, I am here now, what do I need to do to fix it. I hate that some dentists take such a negative tact. We try to keep a positive attitude and congratulate people on taking the first step back to dental health. Definately interview dentists until you find one that will treat you with respect.

  16. QUESTION:
Can you get gum surgery with braces?
For inflamed gums, can they remove the extra gums by doing surgery? If it is possible can you get it with braces? Does the orthodontist or dentist do this?

    • ANSWER:
Yes, you can have sugery done on your gums to remove the bunched up skin. It is also possible to have surgery done with your braces still on, but of course it will depend on the individual.

Generally your dentist would do it or a specialist (ie. someone who does root canals, ect).

  17. QUESTION:
Do most insurances cover getting laser gum surgery?
I had braces when I was younger, 3 years ago (I’m 17 now) and still have overgrown gums that make my teeth look uneven when I smile. I have been self-conscious about it for a long time. I’m wondering if most insurance companies will cover a corrective laser gum surgery even though it’s for all intents and purposes cosmetic.
    how much does it cost?

    • ANSWER:
I don’t think any insurance company would cover this for cosmetic reasons (unless it were due to an accident or something).

The cost depends on what exactly is done and whether there are any additional procedures involved, but usually between 0 and 00 (in the USA).

well buffin of gums after orthodontic treatment is very common findin……..thers nthin to be feared of laser gum surgery or any dentist as dentist is a proffesional n bein doin it for years so have faith in ur dentist……. and as far as laser surgeries are concerned they are less traumatic as theres no surgical openings done and they are the most conservative approach and most advanced approach amongst the surgeries….they have prooved as a boon not only to denstists but for patients aswell as ther is bloodless field or surgery. If u hav encountered the conventional surgeries then definatly u wudnt hav asked such a question!
As they were comparativly painful and bloody!
So have faith in ur dentist and go happily for it without any fear as ur gettin not only more advanced but a bloodless and painless procedure done!

  21. QUESTION:
    How painful is gum surgery?
I haven’t gone to the dentist yet, but will be going soon. I’ve noticed that my gums are definitely not pink. Deep gum pockets have also been formed. After researching this, I’ve come to the horrifying fact that I have gum disease. If I have to get the surgery(which I probably will), how painful will it be? Is it going to be difficult to brush after the surgery?

    • ANSWER:
Please dont worry about the pain part…..dentistry now-a-days is quite painles. Secondly, it is also quite probable that you may not require to undergo gum surgery. Many a times just the scaling i.e scraping away the calculus deposited over the teeth is just good enough to bring back the gums to their normalcy. So, just dont worry and just visit your dentist.
Yes, for about a week after the gum surgery( not in your case, dont worry !!……let’s hope so!!)…….it is little bit discomfortable to do the brushing. But there are ways to tackle that part also….generally the patient is advised to use supersoft tooth brushes during that healing phase or sometimes it is advisable just to use chlorhexidine mouthwash as a “substitute” for brushing …..just for 1-2 days.
Just take care………….but dont worry !!

  22. QUESTION:
Is it normal for gum graft tissue to be loose a week and a half after surgery?

  23. QUESTION:
    How much is gum surgery?
I want to get my gums cut back, because when I smile I have a “gummyliscious” smile. And I hate it. Also, some of my teeth are a bit shorter than they should be.
I’ve been looking into gum surgery but can’t find they costs for it.
    Does anyone know how much?
How long does it take to recover?
I’ve already gone through 5 years of braces.

    • ANSWER:
If you are a young person, I think you need to consult with and orthodontist, and then an oral surgeon. I think you are describing a face/upper jaw/skeletal situation. This can be corrected with both jaw surgery and braces, but it is a big deal and expensive, and would fall under both your medical insurance and your dental insurance.

Also your teeth may be looking shorter because your face/upper jaw/skeletal situation is causing you to wear or grind your teeth unnaturally.

Gum surgery is usually having to do with gum disease in older people, and doesn’t look like what you are describing.

    • ANSWER:

      yogurt
      pudding
      jello

    • ANSWER:

  32. QUESTION:
Has anyone actually gone through perio gum surgery?
Ive seen posts for people contempating the surgery here. I have been advised to have the surgery where they cut open your gums, scrape and sew them up (plainly speaking). I am having deep cleaning 4 times a year, but it seems that even tho I improve a bit, the dentists still like to make referrals to periodontists. They come up with estimates of 00 a quadrant. I would like to hear from actual gum surgery victims :-) about this procedure which they say only helps 50% and lasts only a few years!

    • ANSWER:
I had it done on all 4 quadrants, and my periodontist charged 0 per quadrant. She did a great job, too. I never had any pain. The bone grafts (5 of them, all cow bone) were 0 each, but she gave me a discount so I was charged only 0 each. She also told me that if I continued to take good care of my teeth with regular cleanings (every 3 months) and good hygiene (brushing and flossing), I would never have to undergo any periodontal procedure ever again.

  33. QUESTION:
How hard is it to remove the stitches yourself from gum surgery, as from a difficult tooth extraction?
Parts of the stitches already fell out by themselves. It’s been twelve days since the extraction. I don’t think these are “dissolving” stitches, though. The shattered tooth was an upper molar or something (the big old tooth next to last in the row).

    • ANSWER:
If the dentist wanted you to return for suture removal, then wouldn’t he have told you? Perhaps ring the receptionist and ask.
Dissolving sutures can go anything up to 5 weeks, but that length of time is not really necessary in dentistry.
Most dissolving sutures for dentla use are skin-coloured and will last 5-14 days.
Non-dissolving tend to be distinctly coloured (e.g. blue).

  34. QUESTION:
which one is better scaling or gum surgery?
i have moderate gum problem. periodontics dentist adviced me to go on scaling. but i was afraid that it might wont work then late on advice me to go on surgery. it would be waste of time and money.
so what do you think is better go on scaling or gum surgery.

    • ANSWER:
The most conservative approach should always be given preference. If the periodontist feels that the deep scaling will take care of your problem, then I would follow his advice. It is less traumatic and less costly. But either way you go the key will be for you to continue with good home care and preventive measures. Good luck.

  36. QUESTION:
What does a gum infection from surgery look like?
I just had a tooth exposed 3 days ago and they placed a bracket on it. Since the surgery my swelling is getting worse and my mouth tastes terrible! The gum is all white/grey colored and puffed up so I am worried it may be infected. What are signs of infection?
also…reason im not calling the surgeon is cause hes not there :( hes on vacation for a week! Anything I can do at home?

    • ANSWER:
Generally an infection would present itself as very red and swollen and potentially some oozing. The bad taste in your mouth is not uncommon and you can help both inflammation and taste with some gentle salt water rinsing. If you are still in doubt call the oral surgeon back, they should have someone covering for them (likely another dental practice) and they should check you out. Hope that helps!

  38. QUESTION:
What is the approximate cost for receding gum surgery and does it work?

    • ANSWER:
Skin grafts from the roof of the mouth to the gum area to stabilize the root about 0 per tooth. This skin graft does not cover the gaps that are exposed because of the receding gums (there is no blood supply there to support the transplant), but does give more body to the gum tissue and helps any teeth that may be loose. REALLY uncomfortable. like a bad pizza cheese burn on the roof of your mouth for 5 days.

My mom went to a dentist and was diagnosed with a gum infection. The dentist told her the surgery or treatment costs around 00. 42. QUESTION:
What kind of gum surgery do I need?
My orthodontist said I should get my gum tissue removed. I looked online to see what it’s called so I can find someone to do it, but there is so many different types of surgery! He said I have overgrown tissue and I should get it removed before getting my braces taken off. And that it’s taking away about 25-30% of my smile..Which I don’t want. So Please help me get a better smile!
I’ve looked it up. its actually called a periodontist

    • ANSWER:
You might want to wait until after your braces are off before considering gum surgery (gingivectomy) and the reason is some of my ortho patients had puffy gums that looked overgrown also and the reason was they did not brush properly to keep their gums healthy. When gums are not stimulated sufficiently as happens when you are chewing and eating, they get puffy. With braces on your teeth this natural stimulation is dramatically decreased due to the brackets that are bonded to your teeth. This makes having healthy gums even harder. So if i were you, I would wait until the brackets are removed so that you can brush better and try to get your gums back into shape that way.

Another cause of overgrown gums from my personal experience is the patient being allergic to nickel found in brackets. Once the brackets are removed, the puffy gums will go away. Either way, I think you would be jumping the gun if you had your surgery before the braces are removed. There is always a chance that your gum problem will go away once your braces are taken off.

  46. QUESTION:
are there any ways besides gum surgery to cure gum disease?
like i think i might have gum disease my gums dont bleed but occasionally they hurt and im young so i can ask my mom to take me to the dentist but is there any way besides surgery i dont like the sound of gum surgery

    • ANSWER:
Yes, if you go to a good healthfood store you can get toothpaste with different healing oils in them that heal gum disease. Also get the xylitol mouthwash. Stay away from all products that you might buy at walmart or the grocery store because they contain ingredients that you might be allergic too and actually causing your gums to get weaker.
EDIT:Boron is necessary for cartilage formation and repair; it affects Calcium, Magnesium, and Phosphorus levels. It is essential to bone metabolism and calcification of bones, and helps prevent osteoporosis, arthritis, and tooth decay. Memory and brain function can be improved with Boron.
Boron plays a role in regulating the hormones. Boron is said to potentiate estrogen’s role in building bones by helping convert vitamin D into the active form necessary for the absorption of Calcium. It especially affects estrogen, but testosterone is affected as well. Some research shows that Boron improves libido.

  47. QUESTION:
I’m having laser gum surgery at the orthodontist and i need some tips!?
I know it’s not supposed to hurt because you have numbing on your gums but i was wondering if anyone has gone through it before to tell me what to expect during and and after the surgery! thanks!!

    • ANSWER:
I have never had gum surgery before but I have had the orthodontist use the laser on my lip and it hurt very bad. If you are going to be numb then it should be ok though. It makes a very fast and clean cut. I would think that afterwards you will probably only need advil or something similar and maybe warm salt water rinses.
